Tidepoint Healthcare Staffing is hiring a Travel RN - Case Manager
Shift: Days / 8a-4p, 8x5, 40 hrs/week
Length: 8 weeks
Requirements:
- NY license
- 2 years experience
- NYS PRI Certification
- Screen Certification
- BLS
- Experience with Epic and Care Port transition preferred
Other Details:
- Supports safe and timely patient discharge transition (typically 14-18 patients)
- Coordinates DME and Post acute services
- Identifies high-risk patients and discharge barriers, collaborate with interdisciplinary tea members, documents and coordinates care in Epic
