Tidepoint Healthcare Staffing is hiring a Travel RN - Case Manager - Home Health
Shift: Days / 8a-5p, 8x5, 40 hrs per week
Length: 13 weeks
Requirements:
- OR License
- 2+ Years Experience
- Previous clinical experience in an acute care setting, or home health setting assessing skilled care needs and equipment and health maintenance resource needs
- BLS
- BSN/MSN
- Graduate of an accredited school of medicine, social work, nursing or related medical occupation
- Previous experience with coordinating community resources for post-acute hospitalization care preferred
- Case Management Certification Experience preferred
- Previous Discharge Planning and/or Case Management experience preferred
Other Details:
- CRASE training - required if travel assignment extends beyond a 6-month period
