• MA License
• 1 Year Certified Home Care experience
• 1 Year Case Management experience
• Experience in hospice, private care, or working as a Case Manager on its own does not meet the qualifications for Certified Home Health Care Management experience
• BLS

• Ability to travel to cases as assigned
